Your medical record is kept by the Medical Records, or Health Information Management (H.I.M.) department, here at Westend Hospital.

This information contains things like:

All medical records at Westend Hospital are retained for a maximum of 7 years including digital films from X-rays, and CT scan Dicom images.

You can request copies of your medical records in any of the following ways:

  • Call our outpatient-department mobile number, stating the patient’s full name as it was used during treatment, unit number, and period of care.
  • To allow the sharing of this information, send us a written request via email, together with your medical record number or unit number, full name as it was used during treatment, and your signature.

Within 14 business days, we will handle your request and transfer the records to you using the method you specified. At our Medical Records department, you can also make arrangements to pick up your medical records in person. Monday through Friday, 8 a.m. to 4:30 p.m. are our business hours.

In certain cases, you may be charged for copies of your Medical record, this is dependent on if you have chosen a manual or an electronic record. If a charge is placed, we'll give you a call to let you know the final cost. If you choose to pick up your records in person, you can pay the cost at our payment terminal within the facility.
